Community Transition Nurse (CTN) Mobile Home Base Primary Care is provided by our Board Certified licensed Nurse Practitioner(s). Our locally based Nurse Practitioner(s) provides care for the elderly, disable and individuals in our community with difficulty and challenges leaving the home to see a medical provider. Studies have shown that individuals with difficulty leaving the home only sees a provider in the emergency room.

Our Home-Based Primary Care provides:
